The suggested use of this form is to be used by all teams, ministries, and committees to submit content for Parkside’s Facebook & Instagram.

Submissions must be received at least 7 days before the first requested post date to ensure we have enough time to schedule and create content as needed.

Yes – All individuals (or parents/guardians of minors) shown in the attached photos have given permission for their image to be used on Parkside’s social media. No – I do not have photo consent from everyone pictured. (These photos should not be submitted until consent is obtained.)

Note: By checking “Yes” and submitting this form, you confirm that all required photo permissions have already been obtained. The church assumes that submitted photos are approved for posting.
